Community Nurse

As a Band 5 Community Nurse, you will join our valued Community Nursing Team based at Sheppey Community Hospital, providing care across Sheppey, Sittingbourne and the surrounding villages. This is a rewarding opportunity to deliver high quality nursing care where it matters most, within patients' homes and local communities. You will support patients with a wide range of healthcare needs, helping them maintain independence, manage long-term conditions and avoid unnecessary hospital admissions.

• Delivering safe, effective and person-centered nursing care in community settings
• Assessing patient needs and contributing to care planning and review
• Supporting patients with acute, chronic and complex health needs
• Working collaboratively with GPs, therapists, social care teams and other healthcare professionals
• Promoting patient independence, health and wellbeing
• Maintaining accurate and timely clinical documentation
• Contributing to service improvement and sharing best practice across the team A full UK driving license and access to a vehicle for work purposes is essential.

• Registered Nurse with a valid NMC registration
• Passion for delivering high quality, patient-centered care
• Strong communication and interpersonal skills
• Good organizational skills and the ability to manage a varied workload
• Ability to work independently and as part of a multidisciplinary team
• Flexible and adaptable approach to work
• Confidence using IT systems, including Microsoft Office applications
• Full UK driving license and access to a vehicle for work purposes

• Previous experience within a community, district nursing or primary care setting
• Experience managing patients with long-term conditions or complex care needs
• Understanding of community healthcare services and integrated care pathways, Registered Nurse with a valid NMC registration, strong communication and interpersonal skills, good organizational abilities, capable of working independently and as part of a multidisciplinary team, flexible, adept with IT systems including Microsoft Office, full UK driving license and access to a vehicle for work.
