Position: Care Navigator (Medical Receptionist)

We are currently looking for a Care Navigator to join our friendly and busy Practice. You will be the first point of contact for our patients and will provide a crucial role in providing excellent customer care service. The ideal candidate will be organised, efficient, have excellent communication skills, and be able to work well within a team.

Main duties of the job
  • To be the first point of contact for our patients either in-person or by telephone.
  • Booking appointments for patients.
  • Processing patient prescriptions, both manually and electronically.
  • Processing all incoming emails and electronic letters.
  • Open, sort and distribute all post throughout the Practice.
  • Scanning of documents, and photocopying.
  • Filing of patient notes.
  • Allocation of blood results to GPs.
  • Informing patients of results, and processing results accordingly.
  • Dealing with internal tasks from clinicians, both externally and internally.
  • To use the Accu Rx system for patient communication.
  • Process new patient registrations and changes of patient addresses.
  • Process patient consent forms and online access requests.
  • Liaise with all members of staff within the Practice.
  • Communicate with patients in a polite and courteous manner.
  • Be responsible for the opening and closing of the Practice (on a rota basis).

Job responsibilities

Job Title: Care Navigator

Responsible to:Reception Manager/Practice

Job Purpose: To ensure that the reception area of the Practice runs smoothly, provides a high level of service to patients, doctors, nurses and other members of the Practice team.

Duties and Responsibilities

Telephone Duties:

  • Care Navigating patients to the right clinician when booking appointments.
  • Dealing with queries from Pharmacies, Hospital and other external parties.
  • Communicating results to patients.
  • Ambulance transport bookings including blue light Ambulances.
  • Home visit requests.
  • Deal with general enquiries, compliments and complaints.
  • Contacting patients and other NHS staff.
  • Reception Duties:

  • Taking Registration forms from patients and checking they are accurately completed.
  • Greet and direct patients and visitors.
  • Monitor reception and waiting room areas including making patients aware of late running surgeries.
  • Making appointments as a care navigator and directing to right clinician on the front desk.
  • Opening and dating post and distributing to the right department.
  • Ensure outstanding queries are explained and handed over to next shift, as necessary.
  • Respond to needs of doctors and nurses during surgery.
  • Processing patient request for online services and subject access forms.
  • Collect payments from patients for non-NHS services.
  • Accepting samples from patients and making sure adequately labelled.
  • Prescriptions:

  • Processing patient prescriptions paper and online including repeat dispensing.
  • General:

  • Be responsible for the opening and closing of the premises in the morning and evening including arming and disarming the alarm system.
  • Input and extract information from Practice computer system
  • Always observe health and safety guidelines
  • General housekeeping (e.g. keeping reception and waiting areas tidy).
  • Admin duties to include processing tasks, Accu Rx system, allocation of blood tests to GP's and processing the acute and medication review list.
  • Task allocation by Reception Manager or the Practice Manager.
  • Person Specification Qualifications
    • GCSE grades A to C, preferably in English and Maths
    References
    • Two proven references are required before a confirmation of start date is given, as per NHS employment guidance.
    Experience
    • Excellent communication skills, both verbal and written.
    • Experience working in a customer service role.
    • Experience using Microsoft Office skills, Outlook, Word and Excel.
    • Experience of working in a multi-disciplinary team.
    • Previous NHS experience.
    • Experience of working with confidential material.
