- Establish client eligibility for assessment following referral and determine the required level of assessment
- Conduct complex assessments with clients and carers to fully understand needs and risks
- Develop comprehensive, person-centred care plans in partnership with clients and carers
- Source suitable and cost-effective services using local providers and resources
- Ensure care plans are delivered within budget and remain responsive through ongoing monitoring
- Complete regular reassessments and adjust support plans as circumstances change
- Provide ongoing guidance and support to clients and carers to improve quality of life
- Identify and report unmet needs to the Team Manager
- Maintain compliance with health and safety requirements and diversity policies
- Supervise and appraise team members where required, supporting professional development
- Participate in the team duty system and contribute to internal and cross-department working groups
