We have an exciting opportunity for a Band 5 enthusiastic, motivated, and compassionate nurse to join us in the out of hours team. The service is open seven days a week and your working hours between 8pm and 8am can be discussed at interview.

Your skills will be really valued by our teams and patients alike. If you are new to community health, this is a great opportunity to gain experience in this unique field of nursing.

Main duties of the job

Our team is based on the Countess of Chester health site and provides visits to patient's across the geographical footprint of our eight care community teams during the evening and night.

It's important that you have access to your own transport for work but of course, mileage expenses are reimbursed in addition to your hourly rate.

As a Community Staff Nurse the successful candidate will undertake duties involving the delivery of high-quality nursing services and the holistic assessments of patients ensuring a person-centred approach.

You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.
