Job Summary
The States of Guernsey are looking for a Team Manager to join the Supported Living Team within the Adult Disability Service. The Supported Living Manager at Le Grand Courtil is responsible for leading and managing a high-quality, person-centred supported living service for adults with learning disabilities, autism, and complex needs. The role ensures individuals are supported to live independently with dignity, choice, and control while achieving the best possible quality of life.
The Manager oversees staff performance and development, promotes positive behaviour support, safeguards vulnerable adults, and ensures compliance with organisational and regulatory standards. Working in partnership with families, carers, and multi-disciplinary professionals, the Manager drives continuous improvement, effective care pathways, and community engagement to deliver excellent outcomes for service users.

- Promote and deliver a high standard of care based on best practice to maintain patient/service user safety and provide an excellent standard of care.
- Coordinate the assessment of patients/service users with complex needs, including individuals with unconfirmed diagnosis.
- Coordinate the development, implementation and evaluation of specialised programmes of care utilising evidence and research.
- Lead a team of staff and provide specialist advice in acute and chronic situations, ensuring that the patients/service users’ needs are met along with their support network.
- Ensure patient/service user involvement in the planning, delivery and evaluation of care, respecting their beliefs and dignity.
- Utilise data and information from a range of sources to identify patient/service user condition to guide decision making and, where appropriate, request specific clinical tests to establish a diagnosis.
- Act as a health promoter and educator for patients/service users and their support networks, ensuring professional standards are upheld by staff within the postholder’s sphere of responsibility.
- Manage all incidents as per policy and participate in risk management and Quality and Safety processes (e.g. SWARM), being accountable for ensuring safeguards are in place for patients/service users and escalating any safety, safeguarding or risk concerns following appropriate processes.
- Registered Health or Social Care Professional (e.g., Social Work England, NMC, HCPC)
- Significant experience at a Band 6 or equivalent
- Practice assessor, practice supervisor
- Management Qualification Level 5 and above
- Degree in a relevant field of nursing/management
