Specialist Pharmacy Technician – Healthcare at Home & Community

East of England Community Health and Care NHS Trust (Cambridge) – Closing date 19 July 2026.

We are looking for a highly skilled Band 6 Specialist Pharmacy Technician to deliver patient‑centred medicines optimisation across Community Services in Luton and Bedfordshire. The role supports patients with long‑term conditions, frailty and complex polypharmacy to achieve the best possible outcomes from their medicines.

Key Responsibilities
  • Provide medicines management support to specialist community clinics (e.g. TB, respiratory, diabetes, anticoagulation, heart failure), including medication review, patient counselling, adherence monitoring and identification of medicines‑related risks.
  • Assess, plan, implement and evaluate medicines management interventions in patients’ homes or in community clinics.
  • Maintain accurate documentation of activity, recommendations and outcomes in line with Trust standards.
  • Support safe transfer of care between healthcare settings, addressing medicine‑related barriers to discharge and continuity.
  • Work in partnership with multidisciplinary and organisational personnel within both secondary and primary care, adopting a collaborative approach with patients, their families and carers.
  • Provide specialist advice and practical support to patients and carers regarding medicines use, promote independence and self‑reliance.
  • Devise and deliver structured medication counselling programmes and demonstrations of inhalers, insulin pens and other medical devices.
  • Deliver patient education through group forums and structured sessions.
  • Provide medicines‑related advice and training to MDT colleagues and care providers.
  • Act as liaison between the community and acute sector in relation to the pharmacy pathway.
  • Contribute to admission avoidance, early intervention and care‑home support by proactive medicines optimisation.
  • Support the development and maintenance of Standard Operating Procedures and other relevant documents.
  • Contribute to service improvement, audit and evaluation under the direction of the Specialist Pharmacist.
  • Handle sensitive patient issues, lone working and travel across Luton and Bedfordshire.
Communication
  • Communicate effectively with EEC staff, hospital colleagues, patients, family/carers and community pharmacy and social care teams.
  • Facilitate clear and effective communication channels between patients, relatives, carers and professional colleagues to ensure seamless care.
  • Provide and receive sensitive patient information, taking into account cultural and personal preferences.
  • Maintain accurate clinical records of activities, plans and strategies.
Budgetary Responsibility
  • Support cost‑effective use of medicines and avoid waste wherever possible.
  • Ensure secure and appropriate use of drug stocks and related devices.
  • Minimise medication‑related problems and hospital admissions, and reduce drug costs and waste.
  • Address hoarding of unused medicines and guide appropriate destruction of waste medicines.
People Management
  • Maintain own professional development and participate in appraisal and KSF process.
  • Provide guidance and advice to colleagues within and outside the Trust on safe medicines management.
  • Train healthcare support workers and check transcribing of medication and records by others.
Research & Development Activity
  • Participate in research and audit to identify educational needs and areas for service development.
  • Collaborate with Pharmacy Team to develop and share best practice, integrating research evidence into practice.
Clinical and Practice Governance
  • Work in accordance with Trust policies on medicines management, safeguarding, information governance, infection prevention and equality and diversity.
  • Maintain strict confidentiality and comply with data protection and information security policies.
  • Adhere to Trust risk assessment, risk management, infection control policies and relevant legislation.
  • Maintain safe systems of working as per Medicines Act, Health and Safety legislation and report any defects.
