Job summary

An exciting opportunity has arisen for an enthusiastic and dedicated Clinical Practice Based Prescribing Pharmacist to join our practice team and provide care for our patients at Hattersley Group Practice

Hattersley Group Practice serves around 5,400 patients in a diverse community experiencing significant socioeconomic deprivation and health inequalities. The Clinical Pharmacist will be part of a busy and proactive team working alongside clinical and non-clinical colleagues to innovate and progress the clinical pharmacy service, enhancing patient care and outcomes.

The successful candidate will participate in and support the GP's medicines optimisation to deliver change for the benefit of patients and/or the organisation.

You will be a key member of the senior clinical team being proactive in providing safer prescribing, providing expertise in clinical medicines advice, while addressing both public and social care needs of patients in their care.

The working hours for this post are 37.5 hours per week, Monday to Friday, working pattern to be agreed.

Main duties of the job
  • Provide support to other clinicians with regards to prescription and medication queries/repeat prescriptions, and will demonstrate clinical reasoning and critical thinking, as well as the undertaking of comprehensive structured medication reviews and assessing/processing repeat prescription requests.
  • Provide advice, care and medicines management for patients either via telephone/ video or face to face.
  • Act as a positive role model supporting junior staff whist offering the ability to work flexibly between urgent care and primary care to provide safe, clinical and expert care.
  • Undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
  • Ensure that services integrate with community and hospital pharmacy to help improve patient outcomes, ensure better access to healthcare and help manage workload.
  • Lead on the development of innovation bids, new services or a new mode of service delivery to meet the demands of the changing healthcare environment.
