A position has arisen for a practice nurse to join our team.
We are looking for a motivated, enthusiastic, committed individual to join our friendly and forward-thinking supportive practice. The candidate will complement our established nursing team which currently consists of a nurse practitioner, five practices nurses two of which are community based and two full time health care assistants.
We are a practice that is strongly focused on working together to serve our deprived community and help, where possible, address inequalities.
Both formal and informal communication is an important part of the way our practice runs.
The hours are between 20-25 a week. Due to the nature of the role, hours and salary are negotiable and dependent on experience.
You will be someone who has a passion to join general practice to provide high quality care based on the needs of our population, this will include treatment, screening, prevention care and patient education.
Main duties of the jobSummary of Role - To provide excellent Nursing care and treatment to patients alongside the General Practitioners or independently to agreed protocols and ensure that all nursing care delivered is relevant, of a high standard and is clinically effective for patients and their carers.
You will deliver sensitive information, advice and support regarding illness, treatment and care plans for patients and carers.
You will be responsible for high quality nursing care and management of patients with chronic disease and for developing, implementing and evaluating care.
Offer advice and promote child and adult vaccinations whilst ensuring vaccines are administered under the appropriate PGD.
Provide women's health and family planning services to all patients.
Work as part of a nursing team, support and work with colleagues and manage workloads to ensure resources are managed effectively.
General Nursing Care of the patients - Have a range of clinical skills developed through experience and be competent to deal with the general nursing care according to practice policy and protocols and to refer to a general practitioner as appropriate.
Assist in the provision of nursing care to those patients with chronic disease, according to the practice policy and protocols.
You will also undertake opportunistic screening for patients i.e. BP checks, height, weight and BMI and smoking status.
About usWe are a 7 partner GMS teaching practice in the North of Sheffield with a practice population of approximately 9,600 patients, our team also includes 3 salaried GPs, 5 practice nurses, 2 Healthcare Assistants and a team of reception, secretarial and admin staff.

- Membership of the Nursing & Midwifery Council and as a registered practitioner you are professionally and legally accountable under the NMC code of professional practice.
- Registration qualifications appropriate for tasks and responsibilities.
- Commitment to maintain a high professional standard of nursing care and keep up to date with all aspects of nursing care relevant to the post.
- Be aware of own limitations and experience.
To provide Nursing care and treatment to patients in participation with General Practitioners or independently to agreed protocols.
To ensure that all nursing care delivered is relevant, of a high standard and is clinically effective and to deliver sensitive information, advice and support regarding illness, treatments and care plans for patients and their carers.
To provide appropriate care and follow up to patients and carers within the surgery or their own home.
To advise patients on general health care and minor ailments with referral to the General Practitioners as necessary.
Responsible for nurse management of patients with chronic disease and for developing, implementing and evaluating care i.e. Chronic Heart Disease, Myocardial Infarction, Diabetes, Asthma, COPD and Epilepsy.
Provide women's health and family planning services to all patients.
Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions.
Work collaboratively with colleagues, networks, secondary care, ICB, and local community services and ensure that there is a multi-agency approach to planning and organising care through a multi-disciplinary approach.
AccountabilityAccountable to the Partners and the Management Team.
ResponsibilitiesAccess the nursing needs of patients in the surgery, the community and housebound.
Provide appropriate care and treatment in conjunction with the General Practitioners, or independently, as appropriate with and according to practice policies and protocols.
Maintain effective liaison with other agencies and staff concerned with patient care and with other disciplines within the practice with appropriate regards to confidentiality.
