Senior Community Mental health Nurse

Responsibilities
  • Act as a Named Worker for a designated caseload to ensure service users receive a consistent approach and a named person to contact.
  • Undertake assessments of people referred to the service, completing full needs assessment, risk assessment, and care plan for implementation by a junior worker.
  • Take the lead on unplanned assessments where required.
  • Contribute to a duty system on a rota basis.
  • Deliver supervision to more junior members of staff and facilitate PDR.
  • Contribute to multi-disciplinary service development.
  • Carry out comprehensive assessments of individuals' health and social care needs in a variety of settings and agree the most appropriate course of action.
  • Assess, evaluate and manage clinical risk.
  • Formulate a plan of care based on identified need.
  • Deliver evidence-based interventions as appropriate.
  • Review effectiveness of care plan at regular intervals.
  • Work collaboratively with service users, carers and partner agencies.
  • Maintain computer‑held care records in an accurate and timely fashion, in line with data quality guidance, legislation and policy.
  • Undertake all work in a culturally sensitive manner.
  • Act in accordance with the Community Mental Health Framework.
  • Develop and maintain effective working relationships with Primary Care colleagues.
  • Lead management of individuals with complex needs including mental health restrictions such as 37/41 and CTO.
  • Co‑ordinate meetings, case conferences and act as chair where required.
Supervision
  • Regularly review own caseload with supervisor to promote efficacy and contribute to overall service management.
  • Deliver supervision and facilitate PDR to more junior members of staff.
  • Participate in own PDR process, identifying training and development needs and pursuing relevant actions to maintain and improve competence.
Qualifications and Experience
  • Maintain NMC/HCPC registration / re-validation and adhere to professional guidelines and code of conduct.
  • Work within the framework of relevant policy and legislation both local and national.
  • Take lead of medication management within national and local guidance and policy.
  • Hold professional registration with NMC/RMN.
  • Have experience working in a community setting and with other agencies for service users.
  • Manage complex caseloads within a team setting.
  • Have assessor/supervisor status for working with student nurses or be working towards it.
  • Experience in carrying out report writing for MHA/MoJ.
Education and Training
  • Act as mentor to learners, stay up-to-date with relevant course and curriculum developments, and maintain sign‑off status.
  • Participate in training relevant to the role.
  • Promote insight and understanding of mental health issues to service users, carers, and other agencies.
  • Contribute to audit, research and evaluation as appropriate.
