Specialist Discharge Sister​/Charge Nurse

We are seeking an enthusiastic and driven nurse with excellent clinical and leadership skills to join our Spinal Cord Injury Centre. The Specialist Discharge Sister/Charge Nurse will contribute to the effective coordination and management of discharge planning and the provision of highly specialist discharge advice and guidance in relation to the discharge of patients from PRSCIC, working closely with patients, carers and the wider MDT, ensuring high quality patient experience.

The Sheffield Spinal Cord Injuries Centre is one of the largest SCICs in the UK. We are a 60 bedded (3 ward) purpose-built centre providing comprehensive holistic care to patients with spinal cord injury. The unit provides care to patients, following acute injury, including those patients who require invasive mechanical ventilation assistance, supporting all patients with individualised plans of care to maximise their rehabilitation potential.

The centre also provides a service for patients who require readmission for a variety of surgical procedures including pressure ulcer repair, insertion of baclofen pumps, tendon releases, urological surgery and non-surgical complications related to their spinal cord injury.

• Managing a caseload of patients with complex rehabilitation and discharge needs.
• Coordinating safe, timely, and person-centred discharges in collaboration with patients, families, carers, and the multidisciplinary team.
• Working with Integrated Care Boards, Continuing Healthcare teams, Social Services, community services, care providers, and other agencies to address barriers to discharge.
• Completing specialist assessments and contributing to funding applications and care plans to ensure appropriate support is in place on discharge.
• Leading family meetings, case conferences, and discharge planning discussions, providing clear guidance and support throughout the process.
• Coordinating care packages, equipment provision, housing adaptations, and training for carers and community teams.
• Acting as a specialist resource for discharge planning, offering expert advice to colleagues, patients, families, and partner organisations.
• Monitoring discharge delays, escalating concerns where necessary, and contributing to service improvement and quality initiatives.
• Supporting the Senior Discharge Nurse and deputising when required to ensure effective delivery of the discharge service.
