Newly Qualified Practice Nurse

Job Summary

PCS is looking to recruit a newly qualified nurse to join our friendly and supportive practice team for 18 hours a week. The successful candidate will have the opportunity to complete VTS training. The initial training period will be based at Darnall Primary Care Centre before moving into post at Buchanan Surgery once training is complete. This is an opportunity to develop your skills within primary care working alongside experienced GP's, nurses and wider healthcare professionals.

• Providing a range of nursing services, including patient assessments, health checks, wound care, vaccinations, phlebotomy and preventative health screening.
• Supporting patients with long‑term condition management, including asthma, COPD, diabetes, hypertension and other chronic conditions following appropriate training.
• Promoting health and wellbeing through health education, screening and lifestyle advice.
• Supporting sexual health, family planning, cervical screening and travel health services following training.
• Working collaboratively with GPs, nurses, healthcare professionals and wider community teams to provide coordinated patient care.
• Maintaining accurate patient records and contributing to audits, clinical governance and service improvement.
• Supporting and developing other members of the nursing team where appropriate.
• Maintaining professional standards in line with NMC guidance and taking responsibility for ongoing learning and development.
• Providing a high standard of nursing care to patients, including clinical assessment, screening, treatment, health advice and follow‑up care within professional boundaries.
• Supporting the management of patients with long‑term conditions, including asthma, COPD, diabetes, hypertension, cardiovascular disease and other chronic conditions following appropriate training and competency development.
• Undertaking routine nursing procedures including wound care, wound assessment, phlebotomy, health checks, blood pressure monitoring, urinalysis, BMI checks, health screening and preventative care.
• Supporting immunisation programmes and administering vaccinations and injections in line with appropriate training, patient group directions and clinical guidance.
• Promoting health and wellbeing through lifestyle advice, preventative healthcare and supporting patients to make informed choices about their health.
• Developing skills in additional areas of primary care nursing, which may include cervical screening, sexual health, family planning, travel health, ear care and child health services.
• Providing holistic, person‑centred care, considering the physical, emotional and social needs of patients and referring to appropriate services, including social prescribing where required.
• Maintaining accurate and timely clinical records in line with practice policies, NMC standards and confidentiality requirements.
• Contributing to clinical governance, quality improvement, audits, risk management, infection prevention and control processes to support safe and effective patient care.
• Participating in clinical supervision, training, appraisal and continuing professional development to maintain and enhance clinical skills and knowledge.
• Working within the Nursing and Midwifery Council (NMC) Code of Professional Conduct and ensuring all practice is compliant with professional, organisational and legal requirements.
• Communicating effectively with patients, families, carers and healthcare professionals, providing clear information and support regarding care and treatment.
• Supporting and contributing to the development of the wider nursing team, including assisting with the supervision and support of healthcare assistants and other colleagues where appropriate.
• Participating in practice meetings, service development activities and improvement projects to support the delivery of high‑quality primary care services.
