Our client, a Yorkshire Council, is seeking an Adults Social Worker to join their Hospital Mental Health Team. Fantastic payrate of £30 per hour! Undertake Care Act assessments and develop person-centred support plans for adults with complex mental health needs. Coordinate safe and timely hospital discharges, arranging appropriate care, support, and accommodation services. Complete Mental Capacity Assessments and Best Interest decisions in line with the Mental Capacity Act 2005. Manage safeguarding concerns and undertake interventions to protect adults k collaboratively with multidisciplinary partners, including NHS professionals, families, and community agencies.

Lead on Section 117 aftercare planning and support individuals transitioning from inpatient services to the community. Maintain accurate case records, assessments, and reports, ensuring compliance with legislative and organisational requirements Requirements of this role:
Must be a Qualified Social Worker, registered with Social Work England. Must have minimum of 3 years post qualifying social work experience
