LPN Rotation Requirement

You MUST have phlebotomy experience and if you are hired in this role as an LPN, you will have to participate in an on call rotation to include phone triaging and or occasional home visit depending on the severity of the call. Typically the on call rotation is completed every 11 weeks.

Position Summary

The Clinic Technician is responsible for delivering direct participant care and supporting clinical operations within the Senior Total Life Care (TLC) clinic setting. This includes assisting with procedures, documentation, and coordination of services under the supervision of the Team Clinic RN and the Clinical Services Director.

Key Responsibilities
  • Provide personal care to participants including bathing, dressing, feeding, toileting, transfers, and incontinence care
  • clinical procedures within scope of practice (e.g., venipuncture, EKG, wound care, specimen collection) under supervision
  • Assist with obtaining and documenting vital signs and basic triage assessments
  • Schedule clinic visits per IDT recommendations and support participant navigation throughout the encounter
  • Document care delivery, observations, and changes in condition within the electronic medical record (EMR)
  • Communicate effectively with participants, caregivers, and team members; ensure follow‑up and message closure
  • Support clinic operations including cleaning exam rooms, restocking supplies, and maintaining a safe environment
  • Assist with specialty clinics (e.g., podiatry, dental) and follow‑up on external consults
  • Participate in home services on‑call and weekend rotations for Personal Care Services (PCS)
  • Attend staff meetings and complete annual competencies and evaluations
  • Promote Senior TLC’s mission, vision, and values in all interactions
  • Collaborate with the Interdisciplinary Team (IDT) to ensure continuity and quality of care
  • Perform other duties as assigned within scope of practice
Skills And Competencies
  • Strong interpersonal and communication skills
  • Ability to work effectively in a team and independently
  • Proficient in Microsoft Office and EMR systems
  • Skilled in managing challenging interpersonal situations with professionalism
  • Demonstrates accountability, reliability, and a commitment to participant‑centered care
  • Knowledge of geriatric care and chronic health conditions
  • Ability to lift, turn, and assist adult and elderly participants safely
Minimum Qualifications
  • High School Diploma or GED
  • 1–2 years of experience as CNA II, CMA, RMA, LPN – must have phlebotomy experience and or certification
  • At least 1 year of experience working with frail elderly populations
  • Valid NC license, registry, or certification in field of practice
  • LPN must have completed an accredited nursing program and possess a Nursing license in good standing with the North Carolina Board of Nursing
  • Current unrestricted driver’s license
Preferred Qualifications
  • Experience in geriatric or senior‑focused clinical settings
  • Familiarity with electronic health record (EHR) systems
  • Strong documentation and organizational skills
  • Advanced training or certifications relevant to clinic care
Health Requirements
  • Must be medically cleared for communicable diseases and up‑to‑date on immunizations
  • Must be able to provide care to adult and geriatric populations
