Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties
  • Screening/Assessment – In conjunction with the interdisciplinary medical team, the BHIP Social Worker screens and assesses the Veteran's psychosocial needs and barriers to facilitate access to appropriate VA and community resources. The Social Worker coordinates the gathering of information vital to setting appropriate treatment goals and time frames for achieving those goals.
  • Treatment Planning/Goal Setting – From screening/assessment information, in coordination with the BHIP treatment team, the Social Worker determines realistic goals with the Veteran, including problems, goals, objectives, and specific concrete interventions. Treatment plans are written utilizing the MHS software.
  • Psychotherapy – The Social Worker provides a full range of psychotherapeutic services to utilize the most appropriate evidence-based psychotherapy to provide quality care in the outpatient programs of the Behavioral Health Interdisciplinary Program at the Shelbyville CBOC location. The Social Worker participates in telemental health or similar activities to provide services for Veterans located away from the Medical Center.
  • Referral to Service Providers – The Social Worker arranges referrals to needed VA and/or community resources to facilitate continuity of a Veteran's recovery. The Social Worker collaborates with other service providers in re-assessing the Veteran's needs during the eligibility determination process, coordinating access to information needed for the referral.
  • Resource Usage/Development – As a subject matter expert on services and resources available to Veterans, the Social Worker monitors patterns of resource availability, identifies unmet needs, and communicates with stakeholders to encourage the development of new and innovative resources.
  • Advocacy – The Social Worker advocates for Veterans to ensure that they have access to all needed care and resources within VA and in the community. The Social Worker guides the veteran through complicated enrollment procedures and bureaucratic processes.
  • Crisis Intervention – The Social Worker assesses and intervenes in crises presented by the Veteran and participates in developing crisis management and safety plans to minimize the event of future crises.
  • Education/Health Promotion and Prevention – The Social Worker provides education about VA and community resources, entitlement programs, VA benefits and Advance Directives/Living, completing the advanced directive or living will as requested.
  • Non-patient care responsibilities – The Social Worker enters all Veteran/family contacts in the electronic medical record using appropriate formats and templates, establishes and maintains positive working relationships with employees, volunteers, patients/consumers and other stakeholders within the VA and in outside community agencies, and performs other duties as requested or assigned.
