Independent Optometrist - Walmart

We are looking for an independent optometrist who will lease space within Walmart stores and lead patient care for eye and vision services.

• Launch and manage eye and vision care services in Walmart stores.
• Set practice days, hours, patient schedule, and fee schedule.
• Operate independently as a lease holder, retaining 100% of exam/professional fees.
• Optionally procure equipment/instrumentation and administrative support services, if permitted by state regulations.

• Doctor of Optometry (OD) or Ophthalmologist (MD/DO) from an accredited school.
• Valid state optometry or ophthalmology license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W‑9 form.
