LPN/RN Core Responsibilities
  • Consult and coordinate with healthcare team members to assess, plan, implement and evaluate patient care plans.
  • Prepare and administer (orally, subcutaneously, through an IV) and record prescribed medications. Report adverse reactions to medications or treatments in accordance with policy regarding the administration of medications by a licensed registered nurse.
  • Record patients' medical information and vital signs.
  • Monitor and adjust specialized equipment used on patients, and interpret and record electronic displays.
  • Initiate corrective action whenever the patient displays adverse symptomatology.
  • Provide bedside care for a wide variety of medical patients.
  • Change dressings, insert catheters, start IVs.
  • Prepare equipment and assist physician during examination and treatment of patient.
  • Respond to life‑saving situations based upon nursing standards and protocol.
  • Participate in discharge planning.
  • Utilize time‑management skills.
  • Record all care information concisely, accurately and completely, in a timely manner, in the appropriate format, and on the appropriate forms.
  • Perform other position‑related duties as assigned, depending on assignment setting.
LPN/RN Requirements
  • Current LPN or RN license in the state.
  • Up‑to‑date first aid and CPR certification.
  • At least one year of LPN or RN experience in a Long Term Care setting preferred.
  • Knowledge of Matrix Care preferred.
