• Provide comprehensive primary care services to patients across the lifespan
• Diagnose and manage acute and chronic medical conditions
• Conduct preventive care visits, annual physicals, and wellness screenings
• Prescribe medications and develop evidence-based treatment plans
• Collaborate with physicians and multidisciplinary care teams
• Maintain accurate and timely EMR documentation
• Educate patients on health maintenance and disease prevention

• Education:
  
  Master’s or Doctoral degree from an accredited FNP program
• Certification:
  National certification as a Family Nurse Practitioner
• Licensure:
  Eligible for Connecticut APRN licensure
• Experience:
  
  Prior primary care experience preferred; new graduates welcome
