Somerset Council’s Neighbourhoods Team is recruiting a Social Worker to work with adults in Mendip. You will assess needs, promote independence and coordinate support with partners to achieve positive outcomes in people’s homes and communities.
You will apply strengths-based practice, manage a varied caseload, and work closely with health and community services to prevent unnecessary hospital admissions and support timely discharges.
