An exciting opportunity for a qualified Social Worker to join Somerset Council's Transition to Adult Social Care Team, supporting young people transitioning into adult services. You will hold a caseload of varying complexity, undertaking assessments of risk and care needs, statutory reviews, and carer assessments, while working in partnership with health, education and voluntary agencies. The role involves developing realistic care plans, contributing to multi-agency working, and acting as a link worker across services.
Some experience with young people with Learning Disabilities and transitions into adult social care is desirable. You must hold a recognised Social Work qualification (BA/MA Social Work or equivalent) and current registration with Social Work England, along with access to a vehicle. This is a supportive team offering supervision, post-qualifying training and ongoing professional development.
