Job summary

This vacancy
is for an experienced, caring Pharmacist to join our friendly, established team in
Sherborne.

As a Pharmacist
you will provide expert clinical advice on medicines and polypharmacy. Ensuring
the provision of safe, modern medicines and personalised care. Taking a
proactive role in medicines optimisation, in line with contractual obligations
and expected standards. Support the repeat prescription service and undertake
medication reviews. Deliver an integrated, safe, cost effective and
high-quality pharmacy service, as part of a comprehensive healthcare package
based on our population needs. Inspire
positive and aspirational leadership to the team of pharmacy technicians and
the wider healthcare team in order to maintain a good working environment.

Advertised as full time, there is some scope for the right candidate to work reduced hours across three or four days.

Main duties of the jobWork closely
with the GPs and other healthcare staff to promote good practice in all
prescribing decisions, to develop and implement improved medicines management.
Helping to reduce waste whilst maintaining the focus on patient safety and
well-being.

Share
information relating to pharmaceutical products, medicines management,
medicines optimisation, medicines legislation and general health education so
that patients and colleagues are appropriately informed. You would be expected
to be responsible for an area of chronic disease management.

Support the
general practice staff with prescription and medication queries and help to
enable partnership working with community pharmacies and secondary care.

The post
holder will help to recognise and improve processes, supporting the team to continue
to deliver excellent healthcare. Analysing data to produce reports and
recommendations to support population health management, helping the team to
complete projects through to a successful and timely conclusion.

About us You will
need to be flexible and used to working independently, whilst forming part of a
wider team. Sherborne has a team of three Prescribing Pharmacists, and three Pharmacy Technicians who work the majority of their week at one of our three practices, serving a total patient list of approx. 25,000. You will be contributing directly to the ongoing
health needs in our community, in this vital patient facing role.

Employed by the Sherborne Area Primary Care Network, the successful applicant will work primarily at
one practice, to support the delivery of care to all our patients.

Qualifications

Essential Registered pharmacist

Primary Care Pharmacy Education Program (PCPEP) Independent prescriber IT literate with experience of analysing data, compiling reports and audits

Awareness and compliance with national standards of hygiene, infection control, professional requirements and best practice guidelines

Managing patient consultations Dangerous drug (such as DMARDs & DOAC) monitoring
