The Physical Therapist (PT) provides direct rehabilitation care to patients in accordance with physician referrals, ensuring compliance with all applicable laws, regulations, and Life Care standards.
Education, Experience, and Licensure Requirements- Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT; 2003 or later must have MSPT or DPT).
- Currently registered/licensed in the applicable state, maintaining an active license in good standing throughout employment.
- Preferred:
One (1) year of experience in a post-acute care or related setting. - CPR certification required upon hire or to be obtained during orientation; must remain current during employment.
Job Requirements
- Engage in community health initiatives as appropriate.
- Proficient in Microsoft Word, Excel, and email.
- Collaborate with patients, families, and support departments to meet patient needs.
- Demonstrate good body mechanics at all times.
- Make independent decisions when necessary.
- Knowledgeable about laws, regulations, and guidelines governing post-acute care facilities.
- Implement and interpret departmental programs, goals, and policies.
- Maintain professional relationships with all associates and vendors.
- Uphold confidentiality of all proprietary and confidential information.
- Adhere to company policies, including harassment and compliance procedures.
- Exhibit integrity and professionalism by following Life Care’s Code of Conduct .
- Follow and adapt physical therapy treatment plans for patients.
- Set, assess, and modify realistic, measurable, and functional goals.
- Oversee and evaluate care provided by PTAs, Rehab Aides, and students.
- Document care appropriately and in a timely manner.
- Utilize therapy software accurately.
- Provide excellent customer service and maintain a positive attitude towards patients.
- Assist in patient evacuation when necessary.
- Demonstrate dependable attendance and punctuality.
- Utilize reasoning skills and good judgment in all situations.
- Communicate effectively and work productively within an interdisciplinary team.
- Engage in physical activities such as sitting, standing, bending, lifting, and walking throughout working hours.
- Read, write, speak, and understand the English language proficiently.
