Position: Nurse MDS & Care Coordination Lead (CLC)

The Veterans Health Administration, Department of Veterans Affairs, seeks a Registered Nurse to coordinate care focused on patient education, self-management and satisfaction throughout the continuum of care.

Responsibilities include coding the RAI/MDS sections and CAAs, participating in CLC care plan meetings, educating staff on RAI/MDS processes, supporting quality improvement, and contributing to survey readiness within the CLC.
