Position: LPN Family Practice Nurse Role
Become a Family Practice Nurse with Sherwood Park – Strathcona County PCN and provide vital nursing care. Engage with patients and healthcare professionals to deliver comprehensive support and enhance primary care services.

This position requires a dedicated LPN who will perform various nursing tasks within a family practice setting. You will be responsible for conducting clinical assessments, managing treatment plans, and providing patient education on health maintenance. This role is pivotal in ensuring seamless patient experiences and effective communication throughout their healthcare journey.

Key Responsibilities:

• Administer injections and perform dressing changes

• Support primary care service delivery and patient navigation

• Develop health screening programs and self-management plans

• Ensure accurate record maintenance per practice policies

• Provide assessments and effective patient follow-ups

Requirements:

• Current CLHA registration in Alberta as an LPN

• Minimum three years in primary care nursing

• Standard BLS Provider certificate required
