Overview

Missouri Baptist Medical Center, an acute care hospital in St. Louis County, offers a full continuum of medical and surgical services, including heart care, cancer, women and infants, breast health, gastrointestinal, orthopedic, stroke, therapy, wound and pain management services. The hospital has a 24-hour adult emergency department and cares for pediatric patients at a separate emergency department in collaboration with St.

Louis Children’s Hospital.

Missouri Baptist is the first and only hospital in St. Louis County to be named a Magnet® hospital by the American Nurses Credentialing Center (ANCC). The prestigious designation is the highest credential a health care organization can receive for nursing excellence and quality patient care. The Care Coordination Department is comprised of RN Case Managers, Social Workers, Compliant Documentation Specialists and skilled administrative staff. Case Management in the Hospital setting is a collaborative practice model including patients, nurses, social workers, physicians, other practitioners, caregivers and the community. Case Management encompasses communication and facilitates care along the continuum through effective resource coordination. The goals of Case Management include the achievement of optimal health, access to care and appropriate utilization of resources, balanced with the patient’s right to self-determination.

Preferred Qualifications

Role Purpose

Provides clinical social work services to address the psychosocial needs of patients and families. Provides individual and group counseling, linkage to community resources, expertise in cultural diversity and advocacy. Collaborates with the health care team and ensures timely, cost-effective discharge and transition planning.

Responsibilities

  • Conducts screening and assessment activities to meet the needs of the discharged patient.
  • Applies assessment findings to prepare a patient-centered plan of care.
  • Applies and complies with all regulations and utilizes resources available to patients to manage their continuum of care.
