A local health organization is seeking a Behavioral Health Care Manager to provide care management for CCBHC patients and community members in need. Responsibilities include linking patients to necessary services, conducting outreach, and facilitating communication with healthcare teams. The ideal candidate should have a High School degree, with a Bachelor's degree being optional, and a passion for improving patient health outcomes.

This is a full-time position with a pay range of $23.00-$25.00 per hour.
