Continuum Care of Washington is seeking an Admission RN to plan, organize and direct hospice care in home and community settings. The role requires experienced nursing and a focus on education.
You will perform comprehensive patient and family assessments, develop and revise care plans, administer treatments, coordinate with physicians and an interdisciplinary team, and serve as Case Manager for your caseload.
Schedule:
4 10s, Friday through Monday.
