Ecumen Hospice Twin Cities seeks a dedicated Hospice RN Case Manager to provide skilled nursing and case management in patients' homes and community settings. The role includes an 80-hour bi-weekly schedule, M-F 8am-4:30pm with occasional on-call, one weekend per month, and three holidays per year.
The RN will perform comprehensive assessments, develop and monitor end‑of‑life plans, coordinate with physicians and care teams, and ensure accurate communication of orders. Minnesota license required.
