LINES OF AUTHORITY:
This position is supervised by the Program Director or their Designee. Unless specifically designated this position does not supervise other personnel. The position may require periodic participation in peer review activities.
Advocate for clients, obtain entitlements, coordinate transportation between ChangePoint and other service providers. The BHT/Case Manager is expected to cooperate as a team member in the provision of a therapeutic environment that encourages each customer s emotional growth. This position will be expected to maintain an acceptable level of productivity as measured by billable hours. This position is expected to meet all standards for clinical documentation.
PrincipleDuties/Responsibilities
- Adheres to personnel policies and comply with agency requirements for ethical and professional behavior.
- Provides services to clients as a case manager.
- Is responsible for all aspects of the clinical records, provides crisis services and assist with service coordination for the client.
- Engages and includes the client in the development and implementation of the treatment plan.
- Protects client confidentiality and personal rights.
- Keeps clear, concise and accurate records of clinical activity according to Policy and Procedures.
- Assists clients in obtaining entitlements and benefits pertaining to housing, income, health and other supportive services, as needed
- Provides recreation, leisure, and pro-social skills for clients both individually and in groups
- Provides transportation as needed.
- Work with other agencies to provide wrap services including going in the home and/ or school to prevent out of home placement
- Provides assertive outreach to clients who resist involvement or have difficulty accessing services
- Establishes and maintains a working relationship with personnel and referral resources.
- Perform vocational screenings and evaluations
- Act as a liaison between client, Department of Vocational Rehabilitation, and employer.
- Provide job development and placement services
- Provide community training site development
- Coordinates and accompanies clients to PCP/Doctor appointments.
- Provides health promotion groups including but not limited to hygiene, exercise, and nutrition.
- Must meet state standards for continued education.
- If qualified, provides rotating on-call coverage and update tx plans
- This position requires positive and active support of the company, the client and the co-worker.
- Promotes quality and safety to help maintain a safe and pleasant work environment, follows safety regulations, and actively contributes towards a safe workplace.
- Must participate in quality management and quality improvement activities.
- Performs other duties as assigned by their supervisor or designee.
- Frequent interaction with clients and their support system, usually their family.
- Interface as needed with peers and other clinical team members.
- Collaboration, as appropriate, with other community resources/referral agencies, with the customer s consent.
Qualifications and Experience
- Required:
- High School Diploma
- plus five (5) years Behavioral Health Experience for BHT case management level
- Current Arizona Driver s License.
- Comply with a Drug Free workplace.
- Must be at least 21 years old.
- Knowledge and ability to define the needs of the client served.

Preferred:
- CPR and First Aid certification.
- Knowledge of entitlement process and local resources.
Required:
- Fluent in English, both verbally and in writing.
- Literate.
- Able to effectively employ analytical and problem-solving skills.
