The Wrap Facilitator is responsible for conducting comprehensive strengths-based assessments, developing and overseeing care plans in partnership with team members, and ensuring services are delivered in a family-centered, culturally competent manner. Responsibilities include identifying service providers, facilitating community team meetings, making home visits, monitoring authorized services, and maintaining fiscal accountability. The role also involves providing consultation and education to providers, monitoring progress toward treatment goals, and collecting data for evaluation.
Qualifications:
- Bachelor's degree in social work or related human service field required.
- Minimum of two years experience working with youth and/or families in social services or education; case management and team building experience preferred.
- Experience managing individual service budgets is a plus.
- At least two years of clinical intervention skills demonstrated.
- Skills in fiscal management, team building, and development required.
- Valid driver’s license, auto insurance, and reliable transportation to travel within the region.
Hourly Rate: $19.71 ($41,000 annually)
Supervision for licensure as an LCSW will be provided.
