Enhanced Care Support Worker

As an Enhanced Care Support Worker, you will be an integral part of the Enhanced Care Team, delivering 1:1 enhanced therapeutic care, personal support, and supervision to meet the additional physical and psychological wellbeing of patients. Enhanced care and support will focus on patient‑centred care giving and holistic activity and planning to support the patients’ changing needs during their hospital stay.

This is a relatively new team within the Trust which will develop and evolve with our collaborative approach to meet the advancing demand of the service whilst providing safe, patient care and supervision.

The Enhanced Care Team provides 1:1 patient support and supervision on the wards across both Telford and Shrewsbury hospital sites. The post holder will be under the direction and supervision of ward Registered Nurses and Ward Managers, and will be responsible to the Clinical Lead for Enhanced Care.

• Flexibility in working across shifts, wards and both sites at Shrewsbury and Telford hospitals is mandatory and will be required to meet the needs of patients and service. Working to support other clinical areas will be required at times.
• Provide high‑quality enhanced personal care, support and supervision, respecting people’s dignity and including their individual needs and preferences.
• Provide patient 1:1 care and support through observation and interaction, aiming to reduce the risk of patient harm whilst enhancing patient experience.
• Deliver all patient personal care needs, support, and supervision in a patient‑centred and inclusive manner.
• Plan activities for patient therapeutic stimulation and enjoyment.
• Support new team members.
• Complete a mandatory 2‑week training programme for the fundamental learning of the healthcare service and ongoing support, and additional bespoke training as required to undertake the role proficiently.

Whilst your roster allocation will be known in advance due to the nature of the role and the needs of the service, times of flexibility will still be required. This post will require rotation duties, work on different wards including the Emergency department, and across Trust sites at Telford and Shrewsbury.
