Shropshire Community Health NHS Trust is offering an exciting opportunity for motivated individuals with a passion for the specialist field of complex discharge to work within the Care Transfer Hub based at The Royal Shrewsbury Hospital, with potential for cross-site work at The Princess Royal Hospital, Telford, as and when the service is required.
This post is an ideal opportunity for Nurses, Social workers and Allied Health Professionals, to shape this new service for our patients. The team is expanding and this is an ideal opportunity to be involved in the transformation of the service and work with the existing skilled team in the specialist field of Complex Discharge.
The role and service offer integration and partnership working with team managers, nurses, therapists, health and social care coordinators across both organisations to ensure a seamless service

Main duties of the jobThe Senior Discharge Coordinator holds specialist knowledge in this field and will work within the Care Transfer Hub providing an expert and skilled community focused approach to discharge planning and assessment, embracing the principles of Discharge to Assess Model and providing a person‑centred approach to care.
