Key Responsibilities:
Deliver high-quality nursing care in the community, empowering patients to maintain independence.
Assess, plan, implement, and evaluate person-centred care plans.
Collaborate with multidisciplinary teams, GP surgeries, and acute trusts.
Assist in facilitating smooth hospital discharges and reducing readmissions.
Clinical skill set required:
Complex Wound care including Doppler compression
Simple wound care
End of life care
Medication administration
Catheterisation male female suprapubic
Bowel care
Syringe driver
Insulin administration
Doppler assessment
Compression bandaging
Essential
Qualifications:
To apply , you must have:
NMC registration (valid and active).
A minimum of 6 months of NHS experience as a Community Nurse.
Eligibility to work in the UK.
