About the Role
An experienced Community Psychiatric Nurse is sought to join a Dementia Services team in Shrewsbury, delivering high-quality, person-centred care to people living with dementia and their families/carers within the community. This is a great opportunity to work as part of a supportive multidisciplinary team, providing assessment, care planning and ongoing support to a caseload of patients.

Key Responsibilities

Carry out comprehensive assessments of patients with dementia and associated mental health needs
Develop, implement and review individualised care plans in the community
Provide support, education and advice to patients, families and carers
Work collaboratively within a multidisciplinary team (MDT), including psychiatrists, OTs, social workers and GPs
