Senior Social Worker - Early Intervention & Prevention (Adults)

Telford & Wrekin Council are seeking an experienced Senior Social Worker to join their Early Intervention and Prevention Team. This is a fantastic opportunity to support adults in the community by providing proactive, strengths‑based interventions that promote independence and prevent escalation of need.

• Carry out Care Act assessments, reviews, and person‑centred planning
• Work collaboratively with health and community partners
• Provide professional guidance to colleagues within the team
• Support individuals to achieve positive outcomes through early intervention
• Manage a reduced, well‑structured caseload

• SWE registration
• Strong experience within Adult Social Care
• Driving licence and access to a car (essential)
• Ability to work autonomously in a hybrid model
