Responsibilities
  • Ensure that all necessary written documents—Admission Nursing Assessments, Nursing Notes, Over‑All‑Care Plans, and other required information—are kept current in the resident record system, following facility policies, ASI standards, and State and Federal regulations.
  • Assist the Director of Nursing in maintaining facility standards to meet state, federal, and ASI requirements, and aid with inspections.
  • Serve as a team member in planning, evaluating, and providing a safe home‑like environment and quality of life for residents.
  • Determine that the medication system meets residents’ needs, is an effective administration system, and complies with State regulations and pharmaceutical guidelines.
  • Keep the administrator informed of significant changes in residents’ conditions, transfers to the hospital, expirations, and any pertinent information regarding residents and employees.
  • Assist in determining which residents should be admitted to the facility using established nursing care criteria as a guideline.
  • Make decisions regarding placement of residents and transfer to other rooms as necessary.
  • Communicate with the Administrator, schedule and conduct nursing staff meetings, and maintain records of such. Assess supply and equipment needs for the nursing department and ensure adequate stocks are maintained.
  • Ensure incident reports are completed as necessary or as indicated and review them on a routine basis.
  • Be available for call by staff members on duty concerning nursing care.
  • Perform any other duties as may be necessary as specified by the Administrator of the facility.
  • Complete or participate in assessments including skin, contracture, fall, incontinence, catheter, side rail; complete assigned sections of MDS; create RAP summaries for urinary incontinence, falls, indwelling catheter, feeding tubes, dehydration, oral and dental care, pressure ulcers, psychotropic and physical restraints; review and update care plans as required; complete all required assessments; participate in Medicare meetings and care‑plan meetings.
