The role involves delivering nursing care in patient homes or care facilities in accordance with physician orders and CMS regulations, with coordination with the IDG and R.N.
staff. The LPN will perform ongoing patient assessments, medication reconciliation, and documentation in the EMR, while participating in IDG meetings and maintaining the care plan.
