Job Summary:

Provide skilled licensed practical nursing (LPN) services within a Long-Term Care/SNF setting, ensuring high-quality patient care. Support the health and well-being of residents through meticulous clinical duties and compassionate communication.

Responsibilities:

• Perform comprehensive nursing assessments and develop individualized care plans.
• Administer medications and treatments accurately, following facility protocols.
• Monitor residents' health status and document changes promptly.
• Collaborate with interdisciplinary teams to coordinate patient care.
• Ensure compliance with all safety, infection control, and regulatory standards.

Qualifications:

• Valid state license as an LPN or LVN.
• Proven experience in long-term care or skilled nursing facilities.
• Ability to accurately assess and respond to patient needs.
• Successful completion of COVID-19 vaccination or valid exemption documentation.
• Strong communication and interpersonal skills.
• Ability to wear a photo badge and present identification as required.
