LVN​/LPN – LTC​/SNF

Job Summary:

Seeking a dedicated Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) for a local contract position in a Long-Term Care / Skilled Nursing Facility. Responsible for delivering compassionate patient care on a rotating schedule, ensuring compliance with safety and clinical standards.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Monitor and document patient vital signs, condition, and responses to care.
• Collaborate with healthcare team to develop and implement patient care plans.
• Provide patient education and support to residents and their families.
• Maintain a safe, clean, and comfortable environment for patients.
• Adhere to all facility policies, including  and vaccination requirements.

Qualifications:

• Valid LVN or LPN license in the state.
• Minimum of 1 year experience in long-term care or skilled nursing settings.
• Demonstrate strong clinical assessment and patient care skills.
• Ability to work effectively in a rotating schedule and adapt to changing priorities.
• Proven compliance with healthcare safety standards and documentation protocols.
• Must have completed the COVID-19 vaccination series or have a valid exemption.
