• Deliver direct nursing care to residents in a long-term care setting under supervision, aligning with individualized care plans
• Administer prescribed medications and document administration accurately in the resident chart per physician orders and facility policy
• Perform ongoing resident assessments, monitor vital signs, and promptly report changes in condition to the supervising clinician
• Assist residents with activities of daily living, promoting safety, dignity, and independence
• Collaborate with the interdisciplinary team to develop and adjust care plans; communicate significant updates to families as appropriate
• Maintain infection control standards, safe environment practices, and accurate, timely documentation

• Active LPN license in New Mexico
• Experience in long-term care or geriatric settings preferred
• Ability to work three 12-hour day shifts per week (06:00–18:30) with guaranteed 36 hours per week
• Willingness to participate in a travel assignment and comply with assignment dates
• Strong communication, documentation, and patient-care skills suitable for a high-care needs population
