Care Navigator Supervisor, Day Shift, Inpatient Rehab

AHC Rehabilitation – White Oak Medical Center seeks to hire an experienced Care Navigator Supervisor for our Inpatient Rehab who will embrace our mission to extend God’s care through the ministry of physical, mental, and spiritual healing.

• Lead team to ensure that delays to care are minimized and that effective discharge plans are developed to navigate patients to the most appropriate level of care in a timely manner.
• Support departmental and organizational financial goals of minimizing potentially avoidable utilization and length of stay as well as reducing readmission rate.
• Complete initial discharge needs, psychosocial assessment, and review of clinical needs of patients with focus on ensuring patient’s safe discharge.
• Coordinate and ensure active participation in Interdisciplinary Rounds and other patient care conferences.
• Submit and follow up on Continuing Stay Reviews (CSRs) on patients with non‑Medicare third‑party payors and manage insurance benefit during length of stay.
• Lead delivery of care provided by Care Navigation team to ensure that patients’ needs are addressed and that documentation meets standards.
• Complete monthly rounding and mentoring with assigned staff to facilitate employee engagement and clinical and professional advancement.
• Coordinate all aspects of staffing, scheduling, and patient experience in collaboration with the Director of Care Navigation.
• Collaborate with the Director of Care Navigation to help facilitate the development of staff, systems, processes and PI that drive optimal performance for the department.
• Identify and navigate patient appointments and treatment to reduce barriers to patient discharge; communicate barriers to leadership for resolution and trending.
• Perform other duties as assigned by department leaders.

• Bachelor of Science in nursing or social work required;
  Master’s degree preferred.
• 3–5 years’ experience in patient navigation, discharge planning, or care coordination.
• At least one year of prior leadership experience preferred.
• Maryland RN licensure.
• Licensed driver and ability to operate a motor vehicle; must maintain automobile insurance as directed by AHC.
• Active American Heart Association Basic Life Support (BLS) certification required.

Full‑time weekdays – 70% clinical, 30% supervisory.

$83,112.12 – $
