Position Summary

The Care Coordinator is one of the integral roles of the Integrated Recovery Program (IRP) team. The care coordinator works with other members of an interdisciplinary team to ensure participants have the support they need in order to be successful in their recovery from substance use or abuse. The position provides this support in the form of facilitating linkages to other programs, providing referrals for services internal and external to Mary’s Center, as well as working with pharmacies and insurance companies to ensure participants have easy access to medication assisted treatment.

The position requires an openness to and understanding of co-occurring disorders treatment, trauma-informed care, and a harm reduction approach.

Reportability

This position will report directly to the Addiction Specialist and Care Coordinator Supervisor.

Essential Duties and Responsibilities

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

  • Complete consent for treatment with participants seeking substance abuse support within 24-48 hours of receiving referral.
  • Provide care coordination both internal and external to Mary’s Center in the areas of behavioral health, psychiatry, primary care, dentistry, health specialties, nutrition, public benefits, referrals to education and employment resources, link to housing supports, and additional community resources as needed.
  • Assist Medication Assisted Treatment (MAT) providers and sometimes accompany participants in accessing appropriate level of care including intake at the ARC, detox, and/or inpatient services.
  • Coordinate transportation for participants when necessary.
  • Coordinate the delivery of facilitated telemedicine services for individual participants and/or with partner community agencies.
  • Provide education to participants on how to use Narcan.
  • Work with insurances, MCOs, BHAP, Bup-DAP, and Buprenorphine Drug Assistance Program to ensure participants receive services and medications in a timely manner.
  • Complete insurance Prior Authorizations when necessary.
  • Identify when participants are in crisis and connect to appropriate team member for further assessment.
  • Schedule appointments, make reminder calls, and outreach participants when they no-show for visits.
  • Provide logistical support to program activities, including weekly Dual Diagnosis group, recovery celebrations, and community events (including weekends)
  • Accurately and comprehensively document in the participant’s chart all encounters, contacts, referrals, and other necessary information within 48 hours.
  • Understand and apply state and federal confidentiality regulations with regard to participants’ records and correspondence.
  • Support other sites as needed. Adhere and follow internal program protocols, including but limited to medication and internal workflows
  • Perform other duties as assigned.

Knowledge, Skills & Abilities –

  • Bachelor’s degree OR High School Diploma with 1 or more years of relevant work experience.
  • A thorough understanding of privacy and confidentiality regulations is essential.

Language Requirement – Must have the ability to communicate effectively in English and Spanish as used by participants.

Physical Demands – Regularly required to sit; frequently required to reach with hands and arms, walk, stoop, kneel, crouch, talk or hear; must be able to lift objects up to twenty-five (25) pounds.

Work Environment – Mostly in a typical office and/or telework setting with quiet to moderate noise level. May be exposed to extreme cold, heat, and humidity due to outside weather conditions.
