Human Touch Home Health is looking for Physical Therapists (PTs) for Full-Time, Part-Time, and PRN home health opportunities throughout Rockville and Gaithersburg, Maryland.
We have patient needs throughout both areas and are looking for therapists who want the flexibility of home health without being expected to drive all over Montgomery County.

What You'll Do- Perform comprehensive physical therapy evaluations
- Develop individualized plans of care
- Provide therapeutic exercises and interventions
- Improve strength, balance, endurance, and functional mobility
- Provide gait, transfer, and stair training
- Assess fall risks and home safety concerns
- Help patients regain mobility following illness, surgery, injury, or hospitalization
- Educate patients and caregivers
- Monitor progress and modify treatment plans
- Complete reassessments and discharge planning
- Collaborate with the interdisciplinary care team
- Complete accurate and timely documentation using Well Sky
- Active Maryland Physical Therapist License Required
- Graduate of an accredited Physical Therapy program
- Reliable transportation
- Strong assessment and treatment skills
- Excellent communication and documentation abilities
- Ability to work independently in patients' homes
- Home Health Experience Preferred, but Not Required
- Well Sky Experience is a Plus
