- Develop and manage individualized care plans
- Provide skilled nursing care (wound care, medication management, infusions, catheter care, palliative care)
- Monitor patient status and report changes

- Document care in EHR systems
Active RN License (MD)
- 2–3 years nursing experience (home health preferred; LPN experience considered)
- Strong assessment and clinical skills
- Wound Care Experience Preferred
- Valid driver’s license and reliable transportation
