General Summary of Position Assesses plans, implements and evaluates the quality of care to patients during episodic and chronic visits. Assists physicians in all diagnostic and therapeutic procedures and provides education, counseling and support to patients and their families.

Primary Duties and Responsibilities
  • Assesses the educational needs of patients related to their condition. Develops and/or implements a health education plan for each patient/family unit and for groups of patients.
  • Assists in the development of phone protocols for medical assistants.
  • Assists physician(s) with data collection.
  • Coaches medical assistants to assist patients telephonically including following pre-set protocols.
  • Collaborates as part of the primary medical care team to formulate and document ambulatory care plans for patients and families.
  • Communicates patient's clinical information clearly and accurately in verbal and written format. Assures that medical record is current, accurate and contains appropriate documentation.
  • Completes nursing assessments and conducts ongoing assessments of patients and families. Evaluates patient progress, conveys information to the physician and coordinates appropriate follow‑up actions.
  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with governmental and accreditation regulations. Participates in policy and procedure development and training.
  • Ensures that laboratory specimens are properly labeled, necessary paperwork is completed and specimens are sent to the appropriate laboratory. Documents and files all laboratory results.
  • Implements medical orders and the primary medical care plan for each patient. Dispenses, monitors orders and administers (by mouth, injection or IV) in accordance with physician's plan of care. Performs venipuncture to obtain blood samples for testing and other duties within the standards of practice.
  • Monitors patient phone calls to ensure that patients have an excellent patient experience when contacting the site.
  • Participates in multidisciplinary quality and service improvement teams as appropriate. Participates in meetings, serves on committees and represents the department and hospital/facility in community outreach efforts as appropriate.
  • Performs other duties as assigned. May serve as the primary backup for the duties and responsibilities of other positions at the site.
  • Prepares patients for physical examination and/or specimen collection. Assists physician(s) with invasive and noninvasive procedures and examinations.
  • Processes telephone inquiries, performs telephone triage and documents patient phone calls.
  • Records services and diagnosis on patient's encounter form to facilitate appropriate billing.
Minimal Qualifications

Education

  • Associate’s degree in Nursing from an accredited academic nursing program required.
  • Bachelor’s degree in Nursing from an accredited academic nursing program preferred.

Experience

  • 1-2 years related nursing experience, preferably in an ambulatory health setting or related specialty involving substantial public contact.

Licenses and Certifications

  • RN – Registered Nurse – State Licensure and/or Compact State Licensure. Valid RN license in the District of Columbia, Maryland or Virginia depending on work location required upon hire.

Knowledge,

Skills and Abilities

  • Verbal and written communication skills.
  • Basic computer skills preferred.

This position has a hiring range of USD $35.68 - USD $64.71 /Hr.
