Position Overview

We are seeking a compassionate and dedicated MUST BE A Registered Nurse Case Manager to support clinical operations for our Silver Spring office. This role is ideal for a nurse who is passionate about coordinating patient care, supporting field staff, and ensuring high‑quality, patient‑centered services. As Case Manager, you will play a vital role in fostering communication, promoting patient well‑being, and ensuring compliance with regulatory standards and company policies.

Responsibilities
  • Oversee and coordinate patient care for the Silver Spring office.
  • Provide support, guidance, and communication to clinical staff to ensure high‑quality care delivery.
  • Collaborate with interdisciplinary teams to create individualized care plans that promote patient health, independence, and dignity.
  • Monitor and evaluate patient outcomes, implementing follow‑up or care adjustments as needed.
  • Ensure compliance with all regulatory and accreditation standards.
  • Support staff through ongoing education, training, and professional development.
  • Build strong relationships with patients, families, and community partners.
Qualifications
  • Active Registered Nurse license in Maryland.
  • Minimum of 3 years of clinical experience in home health or a related healthcare setting.
  • Experience In Case Management Or Care Coordination Preferred.
  • Strong organizational, communication, and interpersonal skills.
  • A passion for supporting staff and fostering a collaborative team culture.
