We are seeking a compassionate and dedicated MUST BE A Registered Nurse Case Manager to support clinical operations for our Silver Spring office. This role is ideal for a nurse who is passionate about coordinating patient care, supporting field staff, and ensuring high‑quality, patient‑centered services. As Case Manager, you will play a vital role in fostering communication, promoting patient well‑being, and ensuring compliance with regulatory standards and company policies.
Responsibilities- Oversee and coordinate patient care for the Silver Spring office.
- Provide support, guidance, and communication to clinical staff to ensure high‑quality care delivery.
- Collaborate with interdisciplinary teams to create individualized care plans that promote patient health, independence, and dignity.
- Monitor and evaluate patient outcomes, implementing follow‑up or care adjustments as needed.
- Ensure compliance with all regulatory and accreditation standards.
- Support staff through ongoing education, training, and professional development.
- Build strong relationships with patients, families, and community partners.
- Active Registered Nurse license in Maryland.
- Minimum of 3 years of clinical experience in home health or a related healthcare setting.
- Experience In Case Management Or Care Coordination Preferred.
- Strong organizational, communication, and interpersonal skills.
- A passion for supporting staff and fostering a collaborative team culture.
