Sage Group Health Care Staffing is seeking Hospice Registered Nurse for a 13-week travel/ contract position in Simpsonville, SC. Accepting both local and traveler candidates.

Pay Rate:
Range $2000 to $2080 Weekly Gross pay (Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments
Unit: Home Health, Hospice
Shift: 8 Hours;
Days, 8a – 5p
Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri, No on-call or weekend
Position Requirements- Registered Nurse: 2 years
- Required - Recent/Current Home Health Nursing experience in a home health setting: 1 year within the last 3 years
- Required - Home Care Home Base (HCHB) charting experience
- Required - OASIS experience
- Preferred - Must have valid Driver’s License and Personal Auto Insurance
- Must be Covid Vaccinated orprovide medical or religious exemption
- ASN or an equivalent – Required
- BSN or an equivalent – Highly preferred
- RN License for the state applying or compact if applicable
-Required - BLS Certification from AHA or American Red Cross
- Required
- Provide and coordinate care for assigned hospice patients in comfort of their homes.
- Develop and implement nursing care plans including Start of Care and admissions as assigned.
- Support patient comfort and ability tostay at home - enabling them to spend quality time with their loved ones anddoing the activities they enjoy.
- Empower patients and families to reach healthcare goals by educating them on disease management.
- Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state
Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures. - Ensure physicians’ orders are legible iftaken verbally and transcribed appropriately per policy and procedure.
- Ensure patients’ status is assessed on anongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changesin status.
- Document and complete charting for all patient visits within 24 hours.
- Provide bereavement support to patient’sfamilies as needed.
