Responsibilities for Social Worker/Caseworker:
- Develop individualised care plans based on residents' needs and conditions.
- Implement care plans and monitor residents' progress and care outcomes.
- Develop and coordinate programmes and activities to support residents' care and well-being needs.
- Prepare six-monthly care review reports and conduct monthly interviews for approximately 50 residents.
- Degree or Graduate Diploma in Social Work, Psychology or Counselling.
- Willing to adhere to established Standard Operating Procedures (SOPs) when carrying out duties.
- Willing to work rotating shifts according to the duty roster.
- Minimum 1 year of relevant work experience.
