**** Summary
* * Performs and monitors a variety of duties involved with calculating, preparing, delivering and restocking the infant and adult tube feedings for patients. Calculates, prepares and delivers formulas.
** Job Description
** Demonstrates familiarity with Hazardous Analysis and Critical Control Point (HACCP) guidelines as it pertains to ordering, storing, preparing and delivering infant formulas and adult tube feedings for patients.

Requisition s formula supplies in a timely manner after receiving computer orders and sending appropriate fax orders. Serves as a resource for infant and adult formulas.

Demonstrates strong math skills, accurately calculating the formulas. Exhibits problem-solving skills. Demonstrates excellent customer services skills continuously. Displays listening skills and effectively communicates with co-workers, supervisors and nursing staff. Establishes effective working relationships with co-workers, supervisors and nursing staff. Evaluates and maintains quality control standards.
** Qualifications
* * High school diploma or equivalent preferred.
Possesses ability to read, write and follow oral and written directions. Ability to perform basic math. Depending upon location applying for, a minimum of two years experience in food service preferred. Previous leadership experience preferred. On-the-job training will be provided. Depending on location, may be required to complete Diet Therapy Class and pass Diet Therapy exams.
