American Traveler is seeking an experienced RN Case Manager with an active AK license for an outpatient ambulatory care position requiring recent case management experience.

Requirements
  • Active, unrestricted Alaska RN license required at time of submittal
  • BLS certification required
  • Case Management certification preferred
  • Minimum of 1 year of case management experience in a clinic or hospital setting within the last 2 years required
  • Graduate of an accredited school of nursing
  • Bachelor's degree preferred
  • High school diploma or equivalent required
Responsibilities
  • Outpatient ambulatory clinic setting providing primary care case management services
  • Patient population includes individuals and families across the continuum of care
  • Monday through Friday schedule, 8:00 AM to 5:00 PM
  • 13-week assignment with 40 guaranteed hours per week
  • Floating to other departments or assignments as needed
  • Documentation completed in the facility's electronic health record
  • Performs comprehensive patient assessments and develops individualized plans of care
  • Administers medications and treatments per provider orders and policy
  • Provides patient and family education related to care, medications, and discharge needs
  • Coordinates care with providers and interdisciplinary team members, including referrals and follow-up services
  • Advocates for patients and assists them in navigating the healthcare system
  • Delivers culturally responsive, patient-centered care
  • Exercises independent nursing judgment consistent with Alaska Board of Nursing scope of practice
  • Participates in quality and performance improvement activities and reports adverse events per policy
  • Provides guidance and delegation to ancillary staff and supports orientation and mentoring of staff and peers
  • Start date will be no less than 6 weeks after fingerprint cards are received by the facility
  • Housing and transportation are not provided by the facility
