Are you an experienced community nurse looking to take the next step in your career, or an established Community Matron seeking a new challenge?

We're excited to offer an opportunity for a Community Matron to join our Adult Community Services in Sittingbourne, Kent.

Working within our Integrated Care Teams (ICTs), you'll play a pivotal role in delivering proactive, coordinated care to patients living with complex health needs, frailty and long-term conditions. You'll work closely with multidisciplinary colleagues and partner organisations to help people remain independent, improve health outcomes and reduce avoidable hospital admissions.

This is a highly autonomous role that combines advanced clinical practice, care coordination, leadership and service development, enabling you to make a significant impact on the health and wellbeing of your local community.

As this is a community-based position, a full UK driving license and access to a vehicle are essential.

• Manage a caseload of patients with complex health needs, long-term conditions and frailty
• Undertake comprehensive clinical assessments and develop personalised care plans in collaboration with patients, carers and the wider multidisciplinary team
• Coordinate and review care interventions to ensure patients receive timely, effective and person-centred care
• Support proactive case management and early intervention approaches to reduce avoidable hospital admissions
• Promote self‑management, health promotion and preventative care strategies
• Recognise and respond appropriately to signs of deterioration through effective clinical decision‑making
• Provide clinical leadership, professional guidance and support to colleagues within the Integrated Care Team
• Maintain accurate and timely clinical documentation in accordance with professional, organisational and legal standards
• Contribute to audit, quality improvement activities and service development initiatives
• Act as an advocate for patients, ensuring their needs and preferences remain central to care planning and delivery

We're looking for a compassionate and motivated registered nurse who is passionate about delivering outstanding community care and improving outcomes for patients with complex healthcare needs.

• Current NMC registration
• Significant post-registration experience within community, primary care or integrated care services
• Experience managing patients with long-term conditions, frailty and complex healthcare needs
• Strong clinical assessment and decision-making skills
• Excellent communication, leadership and organisational abilities
• Knowledge of health promotion, prevention and community-based care delivery
• Ability to work autonomously while collaborating effectively within a multidisciplinary team
• Full UK driving license and access to a vehicle
